2012 POOL REGISTRATION

In-person registration: Recreation/Pool Building, 2 Pool and Garage Road
Applications are being accepted Monday - Friday 9:00 am - 5:00 pm (closed Memorial Day)

Mail-In Application: c/o Floral Park Pool, One Floral Blvd., Floral Park, NY 11001
Pre-Season Office Hours: Monday to Friday 9:00 am - 5:00 pm (closed Memorial Day)

Include the following with pool application:

1. Proof of residence - Current 2012 utility bill.
2. Birth of Baptismal Certificate (or copies) must be submitted for each child not a member in 2011.
All documents will be returned with your season passes.
3. Only updated or recent photos 1" x 1" (license size head shot) will be accepted and are required or each and every person listed on
the application with names printed on the back of photos. Please indicate if individual is a senior.
4. Check or money order for the required amount as listed below.

Please note: Applications submitted without proper/complete required documentation will be considered incomplete and will not be processed. Only those submitting complete applications at least two weeks prior to the opening of the pool are guaranteed their passes by opening day.

Please note: FP Fire Department rates apply to volunteers of the Inc. Village of Floral Park only. Items post-marked after June 1^st will be considered late. This village is not responsible for delays in the mail. All exceptions will be dealt with on a case by case basis. There will be no refunds. A "Caregiver Pass" will be handled on a case by case basis. Inquire at the Pool Office for more details.

VILLAGE CODE: There is NO SMOKING at the Floral Park Pool. Smoking must be kept outside the Pool facility.
MEMBERSHIP ELIGIBILITY

Family Membership: Permanent resident families of the Inc. Village of Floral Park on a "first come, first served" basis. Family membership extends to the immediate members of the family residing at the family residence. Immediate members of the family are the father, mother, pre-school children and unmarried full-time students at accredited institutions up to age 23. Married or working sons or daughters, grandparents, or other relatives living at the same address must apply for separate membership either as a Family, an Individual or a Senior Citizen. Birth certificates are required for newborns.
Couple Membership: Married couples who are permanent residents of the Inc. Village of Floral Park. This applies to couples who do not have children that qualify for a Family Membership. This also applies for a single parent with one child. Single parents with more than one child must apply for Family Membership.
Individual Membership: An individual permanent resident of the Inc. Village of Floral Park. Individual Membership does not entitle any other member of the individual's family to the use of the pool or its facilities and gives no privileges to any person other than the individual member.
Senior Citizen Membership: An individual 62 years of age or older by opening day who is a permanent resident of the Inc. Village of Floral Park. Proof of age required for first time seniors.
Guests: The village reserves the right to limit the number of guests and guest privileges. Guests and the sponsoring adult members will be required to register. Proof of the guest's residency is required. Only persons living outside the borders or pool membership eligibility can be guests of Floral Park Pool members. Members are expected to stay with their guests at all times. Fee: $7.00 for guests 7 years of age and over, $5.00 for those 6 years old and under. Limit of 10 visits per guest per season.
